Gyms, clubs, and subscription services are notorious for charges that continue after cancellation. A written cancellation with dated proof of receipt ends the argument about whether, and when, you cancelled.
This letter cancels the membership or service as of a stated effective date, honors any notice period in the agreement, and expressly revokes authorization to charge your card or draft your bank account after that date. It asks for written confirmation, the amount of the final charge, and a refund of anything billed beyond the effective date.
If charges continue anyway, the letter plus the signed Electronic Return Receipt is exactly the evidence your bank or card issuer needs to reverse them.

I am the account holder for the [membership or service] referenced above, opened on or about [start date]. I hereby cancel this [membership / subscription / service], effective [immediately / at the end of the current billing period / on X date, consistent with the notice period in the agreement].
Effective on the cancellation date, I revoke any authorization to charge my card or draft my bank account for this service. Do not process any further charges. If the agreement requires a notice period, this letter constitutes that notice, and the signed Electronic Return Receipt will establish the date it was given.
Please confirm in writing: (1) that the cancellation has been processed and its effective date; (2) the amount and date of the final charge, if any; and (3) a refund of any amounts billed for periods after the effective date, sent to [refund method or address].
If charges continue after the effective date, I will dispute them with my card issuer or bank and file a complaint with the [your state] attorney general's consumer protection office. This cancellation is sent via USPS Certified Mail, and the proof of mailing and signed Electronic Return Receipt will serve as my record of timely cancellation.
