Request for installment agreement documentation: Certified Mail letter template

Send this letter by Certified Mail

Pre-filled template, USPS tracking and Electronic Return Receipt

Taxpayers paying the IRS through an installment agreement often need written proof of its terms: a mortgage lender wants the monthly payment amount, or you want to verify that every payment has been applied to the right tax period.

This letter requests the documents that answer those questions: the current terms of the agreement, a statement of payments received, the remaining balance including accrued penalties and interest, and a payoff amount as of a specific date. Putting the request in writing creates a record if a payment was misapplied and needs to be traced.

Certified Mail with Electronic Return Receipt documents the date of your request, which helps if you need to show a lender or the IRS itself when the information was asked for.

Dear Sir or Madam,

I entered into an installment agreement on or about [date the agreement was accepted] for the balance due on the tax period(s) referenced above, with monthly payments of [monthly payment amount]. I am writing to request written documentation of this agreement for my records.

Specifically, please provide: (1) a copy or written summary of the current terms of the agreement, including the monthly payment amount and due date; (2) a transcript or statement of payments received to date; (3) the current remaining balance, including accrued penalties and interest; and (4) the payoff amount as of [date].

This documentation is needed for [reason: mortgage application, financial planning, verifying that payments have been applied correctly]. If any payment listed in my records, which I can provide on request, has not been applied to my account, please explain the discrepancy in writing.

Please send the requested documents to the address above. This request is sent via USPS Certified Mail, and the signed Electronic Return Receipt will serve as proof of the date of this request.

Sincerely,

Frequently asked questions

Answers to the questions we hear most often about this letter.

Will asking for records affect my installment agreement?

No. This is a records request, not a renegotiation. The agreement continues on its existing terms, and you should keep making every scheduled payment while you wait for the documents.

Can I get some of this information online?

An IRS online account shows balances and payment history for many taxpayers, and account transcripts can be ordered online or by mail. A written request remains useful when you need an official document for a lender or when online records do not match your own.

What should I do if a payment is missing from my account?

Gather your proof of payment, such as bank statements or canceled checks, and ask the IRS in writing to trace the payment and correct the account. Keep copies of everything, including the Certified Mail receipt for each letter.

How is a payoff amount different from my balance?

The balance is what you owe as of today; a payoff amount projects the total through a specific future date, including interest that will accrue until then. Lenders usually want the payoff figure, which is why this template asks for both.
