Request for a copy of your personnel file: Certified Mail letter template
Pre-filled template, USPS tracking and Electronic Return Receipt
Many states give current and former employees the right to inspect and copy their personnel file: performance reviews, disciplinary notices, payroll records, and the documents behind employment decisions. Employers typically must respond within a set number of days.
A written request defines the scope, starts the statutory clock, and creates a record if the employer produces an incomplete file. This template lists the standard categories, offers to pay reasonable copying costs where the law allows them, and asks for written confirmation of the production date.
Requesting the file before a dispute escalates is often the smart move: it shows what the employer documented, and when. Certified Mail with Electronic Return Receipt dates the request beyond argument.
