Credit Report Dispute (to Furnisher): Certified Mail letter template

You can dispute a credit report error with the bureaus, but you can also go straight to the source: the furnisher, the bank, card issuer, or collector that reports the account. Under the Fair Credit Reporting Act, furnishers receiving a direct written dispute generally must investigate, correct inaccurate information, and notify every bureau they report to.
A direct dispute pairs well with bureau disputes and creates its own paper trail. Sending it via USPS Certified Mail with Electronic Return Receipt fixes the date the furnisher's investigation duty began, evidence that matters if the error persists and you escalate to the CFPB or to court, where willful FCRA violations carry statutory damages.

Dear Sir or Madam,
I am disputing information that your company furnishes to one or more consumer reporting agencies about the account referenced above. My credit report from [Equifax / Experian / TransUnion] dated [date of the report] shows the following: [quote the reported information: balance, payment status, dates, account status].
This information is inaccurate for the following reasons: [explain precisely: the account is not mine, the balance is wrong, payments were made on time, the account was closed or settled, identity theft, etc.]. Enclosed are copies of documents supporting my dispute: [list enclosures: statements, payment confirmations, settlement letter, identity theft report].
Under the Fair Credit Reporting Act, 15 U.S.C. § 1681s-2, furnishers of information must investigate direct disputes, correct any inaccurate or incomplete information, and notify each consumer reporting agency of the correction. I request that you complete your investigation, correct or delete the inaccurate information, and send me written notice of the outcome within 30 days.
This dispute is sent by Certified Mail with Electronic Return Receipt. The proof of mailing and the signed receipt document the date your obligation to investigate began, and they will be retained in case escalation to the Consumer Financial Protection Bureau or the courts becomes necessary.
